• Alternatively, please arrive 10 to 15 minutes early to your first appointment to complete the forms at our clinic.

Appointment Cancellations

In the event that you need to cancel your appointment, please be considerate and contact us to reschedule as soon as possible. We reserve the right to charge a fee for appointments cancelled with less than 24 hours notice, as this creates an inconvenience both for us and for other patients.
